In order to receive payment for both Preventive and Sick Visits on the same date of service, Modifier 25 must be applied to the Sick Visit E&M code in order for the payer to recognize that a separate and distinct service was provided in addition to the service provided during the Preventive Visit. If Modifier 25 is not used on the Sick Visit E&M code, then, the payer will “bundle” the 2 services and only pay for the Preventive Visit.
Billing audits assess claims billed for dual-service encounters where claims are audited to determine how often Modifier 25 was either missing, incorrectly applied, or applied without adequate supporting documentation. In situations where the chart does not clearly differentiate the Preventive and Sick Visits from each other, a claim, with or without the application of Modifier 25, can be denied when audited, regardless of whether or not the claim was completed correctly.
